RETURN GOODS AUTHORIZATION FORM

All warranty returns or new product returns are required to have this form filled out, you will be contacted within 24 business hours with a return goods authorization number (RGA). RGA numbers need to be written on the outside of the box and returned to:

All return items are subject to a 15% restocking fee and must be in new condition. Warranties or returns will not be accepted with out a RGA number.
